If you have seen, heard or felt something that did not feel right, you are encouraged to use this form to report it. It might be that you have identified a potential safeguarding issue, think someone's health and/or wellbeing is at risk, or that something has potentially breached a Club policy — whatever it is, please report it so that it can be followed up.
If you feel there is a serious and/or sensitive concern that could have an adverse impact on the operations and performance of the Club — and is something that needs to be escalated quickly — you can use the Raising Concerns policy and process to do so. You can also contact welfare@northwichunited.org.uk.
If you are unsure about reporting, you can speak with Michaela Thompson, Mike Smith or Linzi Hall, who will be able to provide you with guidance.
When you submit this form, it will not automatically collect your details like name and email address unless you provide them yourself, so you can provide an anonymous report if you prefer.
